REQUIRED DOCUMENTS:

  1. W-9 Form (Signed & Dated)

    • Confirms the legal business name, EIN, and tax classification.

    • Must match the name on MC Authority and insurance documents.

  2. MC Authority Letter (FMCSA Operating Authority Certificate)

    • Confirms that the carrier’s authority is active.

    • Can be downloaded from the FMCSA website or provided as a PDF.

  3. DOT Snapshot or Safety Rating Record

    • Confirms DOT number, status, and safety rating.

    • Include a screenshot or PDF printout from FMCSA.

  4. Certificate of Insurance (COI)

    • Must show minimum coverage of:

      • $1,000,000 Auto Liability

      • $100,000 Cargo

    • Policy must list your business name exactly as it appears on the W-9 and MC authority.

  5. UCR (Unified Carrier Registration)

    • Must show current-year registration as active.

    • Brokers often require proof before onboarding.

  6. BOC-3 Filing Confirmation

    • Verifies that a process agent is on file.

    • A copy or filing confirmation email is sufficient.

  7. Voided Check or Bank Letter

    • Used to set up direct deposit for broker or factoring payments.

    • Must show the business name and account details.

Not every broker will request these, but they speed up onboarding and payment setup.

  1. Notice of Assignment (NOA) (if factoring)

    • If you use a factoring company, provide the NOA so brokers can remit payments directly to your factor.

  2. Driver’s License (for Owner-Operators)

    • Confirms identity and matches the business contact.

    • Sometimes requested by brokers for new authorities.

  3. Truck Registration / Equipment List

  • Confirms ownership or operation of equipment tied to your MC authority.

  • Helpful for certain safety questionnaires.
