Include a screenshot or PDF printout from FMCSA.

Certificate of Insurance (COI)

Must show minimum coverage of:

Policy must list your business name exactly as it appears on the W-9 and MC authority.

UCR (Unified Carrier Registration)

Must show current-year registration as active.

Brokers often require proof before onboarding.

BOC-3 Filing Confirmation

Verifies that a process agent is on file.

A copy or filing confirmation email is sufficient.

Voided Check or Bank Letter

Used to set up direct deposit for broker or factoring payments.

Must show the business name and account details.

Not every broker will request these, but they speed up onboarding and payment setup.

Notice of Assignment (NOA) (if factoring)

If you use a factoring company, provide the NOA so brokers can remit payments directly to your factor.
